Option 1: Applying Online

Apply via the CDCP website with your information, or contact CDCP assistance at 1-833-537-4342 if needed.

You’ll need to provide the following information for each applicant, and your spouse or common-law partner (if you have one):

  • Social Insurance Number (SIN) (if available for children)
  • Date of birth
  • Full name
  • Home and mailing address
  • List of dental coverage through government social programs (if applicable)

You and your spouse or common-law partner (if applicable) must have filed your tax return in Canada for the previous year and have received your notice of assessment”

Option 2: You can apply to the CDCP and access CDCP letters using your My Service Canada Account account. Once registered or signed in, go to the “Canadian Dental Care Plan” section on your MSCA dashboard to select “Apply for the Canadian Dental Care Plan”
